Recruiting: National
Summary: HF-REACH is a pioneering nationwide study testing whether nurse led management of heart failure patients is superior to usual care in a community health care setting. Using a smartphone-based mHealth platform monitored by specialist heart failure nurses, who provide core intervention including remote haemodynamic monitoring (weight, blood pressure), symptom triage, patient education, guideline-driven optimisation, algorithm detected decompensation with structured escalation protocols ensuring that all intervention participants receive the same intensity and quality of care study.
GP Requirements:
- Identify eligible heart failure patients.
- Participant recruitment brochures will be provided by CardioNexus.
- Respond to nurse escalations for medication adjustments or clinical review.
- Continue usual care
Participants Recieve:
For practices expressing interest in participation education can be provided on:
- Heart Failure management and guideline-directed medical therapy
- Australian Teletrial Model and Satellite Site participation
- HF-REACH study procedures and how the trial supports your heart failure patients
- Q&A with cardiologist and research team.
Benefits for Your Practice
- Daily remote monitoring of heart failure patients
- No cost for the patients.
- Access to cardiac specialist nursing team
- Early detection of deterioration
- Patient education and self-management support
- Clinical decision support for medication titration
- Primary Site handles ethics, governance, training, consenting, onboarding, nurse-led intervention, data management

Additional Information:
Our point of difference from current community heart failure management in Australia is that we want to focus on community health networks, provide support beyond recent hospitalization and rehabilitation and focus on chronic management of heart failure to support patients who do not have access to specialists on a regular basis. Our IP does this through symptom management, education, medication adherence, and timely escalation to GPs/cardiologists for medical management.
The study uses the innovative Australian Teletrial Model: CardioNexus Melbourne acts as Primary Site providing coordination, training, and support; your practice becomes a Satellite Site conducting the study locally. Activities are delegated based on your capacity. No research experience is required. This novel approach means regional patients can access cutting-edge care without leaving their local community. /
